What information is included on this Site?

Much of the information that is included here is specifically intended for the nephrology professionals who provide renal care in the Network #17 region. Because the mission of Network #17 is to facilitate the improvement of quality of care provided to ESRD patients, a large portion of this is devoted to quality improvement. For instance, there is extensive information pertaining to the quality improvement project "Fistula First."

You will also find materials regarding data collection activities including the use of 2728, 2746, and NPAR forms that are collected by Network #17 as part of its contract with CMS.

In case of emergencies such as natural disasters we encourage providers to see the Disaster Preparedness page and to review the CMS Publication Emergency Preparedness for Dialysis Facilities (pdf - 143 pgs).
