Centers for Medicare
and Medicaid Services (CMS) - Provides information about Medicare
coverage for ESRD patients and Medi-gap insurance policies. Has an
entire section devoted to ESRD
which contains Dialysis
Facility Compare (see description below), Medicare Prescription information, as well as information
about HIPAA.
Clinician
Outreach and Communication Activity (COCA) - COCA was established
by the CDC for the purpose of timely communication of information
on disease outbreaks and terrorism events.
Department
of Homeland Security - Has a section on preparedness and response
to disasters which includes topics such as Training and Technical
Assistance, Publications, and Local Resources.
Dialysis
Facility Compare - This resource is intended to help dialysis
patients easily compare characteristics of medicare-approved dialysis
facilities so that they can make an informed decision about where
to seek dialysis treatment. It includes facility addresses, types
of dialysis offered, and number of dialysis shifts offered after 5
pm. In addition, patients may look at quality measures which include
info about: the percent of patients at a facility with a Urea Reduction
Ratio (URR) of 65 or greater, percent of patients treated with Epogen®
with a hematocrit of 33 or greater, and patient survival information.
End-Stage Renal Disease Network Coordinating Center (NCC) - The NCC provides centralized coordination and support for the ESRD Network Program.
ESRD
Information Resource for Medicare - This is a section of the Medicare
Web site which is devoted entirely to providers of ESRD services. Included
here is the new "ESRD Calculator" which is used for computing
the composite payment rate for patients.

Dialysis Central - Developed by the Medical Education Institute,
this site's goal is to educate patients about home dialysis and its
benefits. Includes a listing of US dialysis centers that offer home
dialysis, descriptions of various types of home dialysis, patient
stories, and discussion boards.
Hypertension,
Dialysis & Clinical Nephrology (HDCN) - This web site offers
web-based learning to anyone on the Internet seeking medical educational
opportunities. Access to lectures, symposia, abstracts, and a variety
of journal articles are available. The learner must register with
the web site to gain access to the material, and must complete a post-test
and evaluation to earn educational credit.
Immunization Action
Coalition - A non-profit organization providing useful and timely
immunization and hepatitis information.
Kidney
End-of-Life Coalition Web Site - This comprehensive site is intended
to promote effective interchange between patients, families, caregivers,
payers, and providers in support of integrated patient-centered end-of-life
care for chronic kidney disease (CKD) patients.
Kidney
School - Part of the Life Options web site, Kidney School
is a web-based learning center that can be used by both professionals
and patients. Kidney School consists of 16 modules addressing a wide
range of topics from treatment options to lab tests. Nurses and other
health care professionals can earn continuing education credits by
going to Kidney School. CE credits can be earned for Modules 1 &
2. Module 1 covers basic information about the causes of chronic kidney
disease, and Module 2 presents treatment options.
Life
Options Organization - Updated and revised in March of 2005, the
new site features an easy-to-navigate home page where visitors can
order or download free materials; find kidney disease information;
read patient tips and stories; and link to "Kidney School".
The site also offers a downloadable Licensing Agreement which allows
users to print and distribute copies of any requested booklet on the
site. The Agreement also allows users to "co-brand" the
booklets by adding a name, address, and "this copy compliments
of _____" in a 3-inch-square box on the lower front or back cover.
MedWatch
- As part of the FDA Safety Information and Adverse Event Reporting
Program, the MedWatch web site reports medical product safety alerts,
recalls, withdrawals, and important labeling changes. MedWatch also
allows health care professionals and consumers to report serious problems
that they suspect are associated with the drugs and medical devices
they prescribe, dispense, or use.
National
Institute of Diabetes, Digestive, and Kidney Diseases - The NIDDK
web site provides information about many different health topics, current
research and clinical trials, advances and initiatives, grant application
procedures, and provides many useful links.
National
Kidney Foundation - Information about advocacy, support groups,
education programs and pamphlets about kidney disease, kidney failure,
dialysis and transplant.

- A plethora of information including a listing of closed dialysis
units during disasters, links to journal articles, current news stories,
a GFR calculator, a food value calculator, patient education articles,
and lots more. Highly recommended.

Promoting
Excellence in End-of-Life Care is dedicated to long-term changes
to improve health care for dying persons and their families. Particularly
targeted are specific diseases including End-Stage Renal Disease.
This site contains the final report summary that the ESRD Workgroup
compiled regarding the need for palliative care in the ESRD population.

- Created by the Federal government, this site covers all aspects
of disaster preparedness and has links to community and state information.

- This site was created to provide a clearinghouse for information
on the cause, treatment, and management of kidney disease and ESRD.
RenalWEB.com
- Lots of information about dialysis including: current news, dialysis
yellow pages, discussion boards, legislation watch, meetings and conferences,
and hundreds of links to other dialysis resources.
RxAssist.org
- This site is a compilation of application criteria and forms for
use by health care providers applying for free pharmaceuticals for
their uninsured patients. Most drug manufacturers offer a limited
supply of free prescription medication to eligible patients.
Scientific
Registry of Transplant Recipients (SRTR) - The SRTR designs and
carries out rigorous scientific analyses of data and disseminates
information to the transplant community.

Network for Organ Sharing (UNOS) - This agency manages the national
waiting list for organ transplants, and the web site is undoubtedly
the best place to start when looking for transplant information.

States Renal Data System (USRDS) - A national data system which
collects, analyzes, and distributes information about ESRD in the
United States.
