The Centers for Medicare & Medicaid Services (CMS) has a page on their website called Dialysis Facility Compare (DFC) which can locate Medicare-certified dialysis facilities based on a state, city, or zip code. The DFC page also provides other information about facilities including types of services offered and quality of care statistics.
